Elective Surgery COVID-19 Screening

In response to the COVID-19 pandemic, the Department of Health & Human Services has developed a COVID 19 Surgical Booking Screening Checklist. This checklist MUST be completed both before and on the day of a patients admission into hospital. This is to ensure patients are screened appropriately and any potential risks are identified prior to their admission. Our pre-admission nurse will call you in the lead up to your admission to complete the initial Covid screening checklist. This will be performed again by your nurse upon admission.
